Hazard Contact

Hazard Contact(formerly Spitz Contact) is a very simple contact form script that you can use as a contact form on your site. It is totally customizable and template driven. The visitor never sees your email address throughout the process. Now it includes a validator which checks that all fields are not empty and that the email address is valid.
