hwdePOLL

hwdePOLL solves the problem with the form processing in a different way. You have the opportunity to create a questionaire with more than one HTML file. You decide when all the data will be sent to you in a nice HTML email. Meanwhile the data will be stored in session variables. No need for a mySQL backbone. Define as many fields as you want and name them as you like. hwdePOLL sends all the data at the end of your questionaire to your email address. There are 4 hidden variables available to control the behaviour of the script. You also can record the IP address, Hostname, etc. from your visitors.
